In an effort to ensure that students are in school, a joint decision has been made by the New Haven Public Schools and the New Haven Health Department.
Connecticut state law requires that all students new to the district, as well as students entering grades kindergarten, 7 and 10 must have an updated physical examination and up-to-date required immunizations in order to attend school.
If we do not receive documentation of a current physical and immunizations by September 30, 2022, your child will be excluded on Monday, October 3, 2022.
After September 30, please do not send your child to school without proper documentation. We will not allow him or her into class. You will be called to pick your child up.
Please make an appointment with your child’s health care provider to be completed as soon as possible. Your child may be permitted into class if an appointment has been made and an appointment card or doctor’s note is submitted verifying the appointment date and time. However, no appointment cards will be accepted after September 30.
The Health Department is offering immunizations and physicals. If you are unable to get your child to a doctor, please call the Health Department at (203) 946-6999. You may also call the following clinics to get an appointment:
